Test performed to assess the integrity of the supraspinatus muscle and supraspinatus tendon. The test is performed with the arm elevated to 90 degrees, the shoulder is medially rotated and angled forward 30 degrees (empty can position) with the thumbs pointing toward the floor in the plane of the scapula. Test carried out to confirm the presence of a distal biceps tendon rupture. The patient abducts the shoulder to 90 degrees with the elbow flexed to 90 degrees and the arm supinated so that the thumb faces up. The patient then actively supinates the forearm against resistance of the examiner, with the index finger of the other hand the examiner attempts to hook underneath the biceps tendon and if no cordlike structure can be hooked, the test is positive for a rupture of the distal biceps.
